Treatment with Transjugular Intrahepatic Portosystemic Shunt(TIPS) in Esophageal Variceal Bleeding



Abstract
Purpose:
@EN To report our experiences of transjugular intrahepatic portosystemic shunt(TIPS) for the control of variceal bleeding.
@ES Material and Methods:
@EN Nine patients underwent TIPS creation for variceal bleeding. All patients are male with liver cirrhosis. Three patients show substantial volume of ascitic fluid. Most patients have been treated with multiple endoscopic sclerotherapy or
ligations for
multiple episodes of bleeding in the past. Measurement of hepatic and portal vein pressure was performed before and after the procedure, and all patients were followed up at regular interval after TIPS creation.
@ES Results:
@EN Shunts were successfully completed in all nine patients. No serious procedure-related complication was noted. Portal vein pressure was reduced from 29.6¡¾6.0mmHg to 19.2¡¾6.7mmHg. The residual portohepatic pressure gradient was 8.2¡¾4.1mmHg.
Substantial decrease in ascites is noted after TIPS. Three patients developed hepatic encephalopathy after the procedure. It occured at the 9th, 40th, and 320th days, respectively. During the follow-up period(3-14 months), all patients have
survived,
and recurrent bleeding was not developed in any patient.
@ES Conclusion:
@EN TIPS is effective in lowering portal pressure and controlling variceal bleeding, particulary in patients with acute variceal bleeding unresponsive to the sclerotherapy and patient contraindicated for surgical shunt operation.
